DUI Services
All court-ordered services begin with an assessment to determine appropriate treatment.
Before scheduling your assessment, you will need to provide us with the following:
Court Order
BAC Results (Blood Alcohol Content) if you do not have this, your attorney or the police station can provide it to you
Arrest/Incident Report
Photo ID
Completed patient paperwork (see form below)
ADOT screening/assessments - $250
ADOT Revocation - $250
Following your assessment, you will be enrolled in the required treatment.
AZ State statutes require either 16 or 36 hrs of treatment. 16 hr classes are split into 2 Saturdays per month and cost $200 per day. The additional 20 hrs are split into 5 classes of 4 hrs each and cost $100 per class.
